Q: What is the Well-Being Assessment?

A:

The Healthways Well-Being Assessment™ (WBA) is a confidential questionnaire that guides you through a series of questions to evaluate your physical, emotional and social health and how your lifestyle habits affect your overall well-being. It takes approximately 20 minutes to complete.  The answers you provide will not be shared with your employer or SHBP.

A:

The Well-Being Plan is a personalized summary of your overall well-being. Q: Why should I participate?

A:

Your well-being is important. With the right information, you can be better prepared to address any health risks that could possibly affect the quality of your life. The Well-Being Assessment will help you to understand the state of your well-being so that you can take the necessary steps to improve it.  All SHBP members and covered spouses covered under non-Medicare Advantage BCBSGa or UnitedHealthcare who complete their Well-Being Assessment™ (WBA) and a screening (on-site or using a 2017 Physicians Screening Form) will earn 240 well-being incentive credits.  You must complete your WBA before any incentive credits will be awarded. Dependents 18 or older are not eligible for the incentive.
