WHO CAN PARTICIPATE IN THE PROGRAM?

Members and their covered spouses enrolled in Anthem and UnitedHealthcare Commercial (non-Medicare Advantage) Plan Options can participate in the Be Well SHBP well-being program and earn incentives.

The 2024 incentives do not apply to dependent children or members enrolled Kaiser Permanente and Medicare Advantage Plan Options.

Dependent children age 18+ can complete certain program activities. Please refer to the chart below to see what you can participate in.

CAN I UPDATE ANY MISSED DAYS FOR A MONTHLY CHALLENGE?

Yes, you can go back seven days and update trackers. The calendar allows you to view tracking trends for each health category over time to see which areas were in the green. Selecting a date within the calendar will allow you to manually add tracking data for that day. Click here to learn more.

DO I HAVE TO PHYSICALLY REDEEM MY POINTS?

Yes, you must physically redeem your points by December 15, 2024. You can redeem them for well-being incentive credits, in increments of 120, or for the Sharecare Rewards Visa® Prepaid Card. If you do not make a selection by December 15, 2024, your points will be redeemed as well-being incentive credits.

HOW DO I REDEEM MY POINTS?

To redeem your points follow the instructions below:

  • From your computer:
    • Go to BeWellSHBP.com and follow the prompts to login to Sharecare.
    • Choose the “Achieve” icon on the left side from the menu.
    • Choose the “Rewards” option and click on the SHBP 2024 Rewards Program.
    • Select the Rewards tab and click Redeem My Points. Make sure you have enabled pop-ups.
    • Select the reward of your choice (well-being incentive credits or Visa Prepaid Card) at the bottom of the page and add to your cart.
    • Click Add to cart and fill out your information to complete your order.
  • From your Android or Apple smartphone using the Sharecare app:
    • Open the appChoose the “Achieve” icon from the bottom menu choices. This will open up additional menu options.Choose the “Rewards” option and click on the SHBP 2024 Rewards Program.Select the Rewards tab and click Redeem My Points.
    • Select the reward of your choice (well-being incentive credits or Visa Prepaid Card) at the bottom of the page and add to your cart.
    • Click Add to cart and fill out your information to complete your order.

HOW CAN I DELETE INFORMATION FROM MY HEALTH PROFILE?

If you feel that certain records are inaccurate or if you just want to remove them, you always have the option to remove any records with the Add/Delete functionality. You can also add information you want included in your Health Profile. Currently you can delete conditions, immunizations, medications and procedures. You must go to BeWellSHBP.com and log in to access your Health Profile. The Add/Delete functionality is also available through the Sharecare App (iOS and Android). Please see some example screens below for removing unwanted Health Profile records. If you have questions or need assistance, please call Sharecare at 888-616-6411.

I AM A KAISER PERMANENTE (KP) MEMBER. CAN I PARTICIPATE IN A SCREENING?

You are NOT eligible to participate in 2024 biometric screenings provided by Sharecare and the screening vendor, Quest Diagnostics. Kaiser Permanente is offering screenings for KP members. All biometric screenings and incentives administered by Sharecare are only available to members and their covered spouses enrolled in Anthem and UnitedHealthcare Commercial (non-Medicare Advantage) Plan Options. RealAge® Test

IF I DON’T FINISH TAKING THE REALAGE TEST, DO I HAVE TO START OVER?

No, you can pause the RealAge Test at any time and all your answers are automatically saved so you can complete the RealAge Test at a later date.

HOW DO I REVIEW MY REALAGE RESULTS?

You can access your RealAge Test results through your Health Profile under the “You” section. There, you will find personalized recommendations and you can update your answers at any time to get a more accurate, updated RealAge.

WHAT IF I DON’T HAVE ACCESS TO A COMPUTER OR A SMARTPHONE TO COMPLETE MY REALAGE TEST?

Please contact Sharecare member services at 888-616-6411 and we will work with you on another option.

WHO CAN TAKE THE REALAGE TEST?

The RealAge Test is available to all SHBP members, covered spouses and dependent children 18 or older enrolled in Anthem and UnitedHealthcare Commercial (non-Medicare Advantage) Plan Options. Note: Dependent children 18 or older can take the RealAge Test and participate in well-being coaching, but they cannot earn incentives. Kaiser Permanente and Medicare Advantage members are not eligible to participate in the Be Well SHBP well-being program.

IS MY HEALTH INFORMATION PROTECTED?

Your privacy is ensured in compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996.

Biometrics

HOW MUCH DOES A BIOMETRICS SCREENING COST?

There is no additional cost for SHBP members and covered spouses to complete their biometric screening in one of three ways:
• Attend an SHBP-sponsored onsite screening event
• Get screened at a Quest Diagnostics Patient Service Center (PSC)
• Have your physician complete a 2024 Physician Screening Form*

*If you have your screening completed at the same time you receive other medical services or treatments (for example, a sick visit), you may have to pay co-insurance/co-pay. Some physicians might charge a fee to fax the Physician Screening Form to Sharecare. You can obtain a copy of your completed form and directly fax it to 615-349-9111 or upload it to Sharecare.

WHO ADMINISTERS THE SHBP-SPONSORED ONSITE BIOMETRIC SCREENING?

Onsite screenings are administered by Sharecare and their screening vendor, Quest Diagnostics. Sharecare protects the confidentiality of your information in compliance with the requirements of the Health Insurance Portability and Accountability Act (HIPAA) of 1996 and applicable state laws.

HOW DO I PREPARE FOR THE SCREENING?

Here is how you can make sure to have a successful screening experience:

  • Although not required, we recommend fasting for at least nine hours prior to your screening; this includes all food and drinks except for water.
  • Failure to properly fast may result in inaccurate blood test results. (If you have a health condition, such as diabetes or pregnancy, or any other medical concerns, consult a doctor before fasting.)
  • Drink plenty of water to make it easier to give a sample of your blood.
  • Continue taking medications as prescribed the day of the screening.*
  • Arrive five minutes before your scheduled time.
  • Allow about 20 minutes for your screening.
    *Follow your doctor’s instructions for taking your medications, including taking them with food if necessary.

WHAT IF I DON’T SEE MY BIOMETRIC SCREENING RESULTS POSTED IN MY HEALTH PROFILE?

It can take between two to four weeks before you will see your results populated in your Health Profile. If you used a 2024 Physician Screening Form, it is also dependent on when your physician’s office faxes the results to Sharecare. To see your results, follow the below instructions:

  • From your computer:
    1. Go to BeWellSHBP.com and follow the prompts to login. (If you have not yet registered and completed your RealAge Test, you will need to do so at this time.)
    2. Choose the “You” option from the menu choices on the left. This is the last icon with the person in the circle.
    3. Click on the Health Profile to see your results.
  • From your Android or Apple smartphone using the Sharecare app:
    1. Open the app and follow the prompts to login. (If you have not yet registered and completed your RealAge Test, you will need to do so at this time.)
    2. Choose the “You” icon from the bottom menu choices. It is the fifth option from the right.
    3. Click on the Health Profile to see your results.

WILL I BE PENALIZED FOR MY BIOMETRIC SCREENING RESULTS?

CAN I GET CREDIT FOR LAB WORK DONE IN LATE 2023 IF MY PHYSICIAN USES A 2024 PHYSICIAN SCREENING FORM?

No. The screening must be completed, and the form completed and submitted to Sharecare between January 1, 2024 and December 2, 2024.

Preventive Screening

WHAT ARE THE DIFFERENT PREVENTIVE SCREENINGS THAT QUALIFY FOR POINTS?

The following preventive screenings qualify for points: colonoscopy, mammogram, pap smear or prostate screening.

HOW MANY POINTS IS EACH PREVENTIVE SCREENING WORTH?

Each preventive screening is worth 60 points.

HOW MANY PREVENTIVE SCREENINGS CAN I COMPLETE TO EARN POINTS?

You can earn points for completing up to two (2) preventive screenings in the 2024 program year, for a maximum of 120 points total.

WHAT’S THE DIFFERENCE BETWEEN GETTING POINTS THROUGH CLAIMS VS SELF-ATTESTING TO THE PREVENTIVE SCREENING?

  • For screenings completed from January 1, 2024 – August 31, 2024, points will be assigned automatically once the claim for the screening comes through.
  • For screenings completed in September, October, November and December 1 or 2, 2024 ONLY, members can self-attest by December 2.
  • For screenings completed from January 1, 2024 – August 31, 2024, points will be assigned automatically once the claim for the screening comes through. For screenings completed in September, October, November and December 1 or 2, 2024 ONLY, members can self-attest by December 2nd. IMPORTANT: Please do not self-attest to any screenings completed in January-August 2024.

HOW LONG DOES IT TAKE FOR THE POINTS TO SHOW IN MY ACCOUNT?

  • Providers have up to 90 days to submit claims. If it has been over 90 days since your claim processed and you don’t see the points in your account, please call Sharecare Member Services at 888-616-6411 to check on the claim status.
  • For screenings completed in September, October, November and December 1 or 2, 2024, once you self-attest to the screening, the points should show in your account within 24 hours

WHAT IF I JUST HAD A PREVENTIVE SCREENING AT THE END OF 2023? CAN THAT BE COUNTED?

Only screenings completed during the 2024 program year (January 1 – December 2, 2024) will count toward points.

MY SCREENING IS NOT SCHEDULED UNTIL DECEMBER 2024, WILL THAT BE A PROBLEM?

DOES THE PSA (PROSTATE SPECIFIC ANTIGEN TEST) COUNT AS A PREVENTIVE SCREENING EXAM?

Yes, the PSA qualifies as a preventive screening exam.

WHAT IF I GO IN FOR A PREVENTIVE SCREENING AND IT BECOMES A DIAGNOSTIC SCREENING, AS REQUESTED BY THE HEALTHCARE PROVIDER?

You will still earn points if you go in for a preventive screening and it becomes a diagnostic screening.

WHAT IF I DON’T QUALIFY FOR A PREVENTIVE SCREENING BECAUSE OF MY AGE OR ANOTHER REASON? CAN I RECEIVE A MEDICAL EXEMPTION?

Medical exemptions will not be offered for preventive screenings. Please review other wellness activities to reach the maximum points in 2024 (480 points):

Members can take the RealAge Test (120 points) and complete a biometric screening (120 points), and members can earn a maximum of 240 points by completing the following activities in any combination:

  • Well-being coaching (40 points for each completed coaching call per calendar month, up to 6 times)
  • Online monthly challenges (40 points each, up to 6 times)
  • Preventive screening exams (60 points for each completed screening exam, up to 2 times)

IS MY HEALTH INFORMATION PROTECTED?

Your privacy is ensured, in compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996. On each phone call, you will be asked to verify your full name, address and date of birth to ensure Sharecare is speaking with the correct member.

WHAT ABOUT HEALTH COACHING FROM MY HEALTH PLAN?

You can participate in the Sharecare well-being coaching program in addition to any health coaching you may do through Anthem or UnitedHealthcare. Sharecare well-being coaching focuses on lifestyle changes, while your Health Plan coaching program is available to help you manage chronic health conditions. Only Sharecare’s well-being coaching helps you earn up to 240 points.

CAN MY SPOUSE EARN POINTS TOO?

All SHBP covered spouses are eligible to earn up to 480 points. That is a family total of up to 960 points. Note: Covered dependent children are not eligible to earn incentives.
